As a Social Prescribing Link Worker, you will play a vital role in empowering individuals to take control of their health and wellbeing by connecting them with community resources and services.

Key Responsibilities:

  • Work closely with GP practices and other healthcare professionals to identify individuals who would benefit from social prescribing.
  • Build trusting relationships with individuals, their families, and carers to understand their needs and priorities.
  • Develop and implement personalized support plans to help individuals achieve their goals and improve their health and wellbeing.
  • Collaborate with community groups and organizations to provide referrals and support to individuals.
  • Monitor and evaluate the effectiveness of social prescribing interventions and make recommendations for improvement.

Requirements:

  • Experience of working in a community development context, adult health and social care, or a related field.
  • Knowledge of community development approaches and the ability to work with diverse communities.
  • Excellent communication and interpersonal skills, with the ability to build trusting relationships with individuals and communities.
  • Ability to work independently and as part of a team, with a flexible approach to work.
  • Knowledge of IT systems and ability to use word processing skills, emails, and the internet to create simple plans and reports.
