Social Prescribing Link Worker

We are seeking a highly skilled and compassionate Social Prescribing Link Worker to join our team at Alliance for Better Care CIC. As a key member of our team, you will play a vital role in empowering individuals to take control of their health and wellbeing by connecting them with community resources and services.
About the RoleThe successful candidate will work closely with our GP practices, community groups, and statutory services to provide personalized support to individuals, their families, and carers. You will work from a strength-based approach, focusing on the person's assets and priorities, to co-produce a personalized support plan that addresses their unique needs and goals.
Key Responsibilities- Build trusting relationships with individuals, their families, and carers, providing non-judgmental support and respect for their diversity and lifestyle choices.
- Work collaboratively with community groups, statutory services, and other agencies to ensure seamless referrals and support.
- Develop and maintain a deep understanding of the local community, including its strengths, challenges, and resources.
- Provide leadership and support to community groups, helping them to develop and sustain their services.
- Work closely with our GP practices to ensure that individuals receive timely and effective support, and that referrals are made to the most appropriate services.
To be successful in this role, you will need:
A strong understanding of the wider determinants of health, including social, economic, and environmental factors.Experience of working in a community development context, adult health and social care, learning support, or public health/health improvement.Excellent communication and interpersonal skills, with the ability to work effectively with individuals, community groups, and statutory services.A commitment to reducing health inequalities and promoting health and wellbeing in the local community.We are looking for a highly motivated and compassionate individual who is passionate about empowering individuals to take control of their health and wellbeing.
