Our 10-year strategy aims to end homelessness across Great Britain, and we're seeking a skilled professional to join our team as an Intensive Case Management Lead Worker.
About the Role
As an Intensive Case Management Lead Worker, you will work with individuals experiencing homelessness and complex needs, providing person-centered support to help them achieve their goals. You will conduct thorough assessments, develop tailored plans, and work collaboratively with members to address their challenges. Your holistic approach will prioritize the well-being and empowerment of each individual, ensuring they receive the support they need to end their homelessness.
About You
We're looking for an experienced and driven professional to join our team. You will have a strong background in case management, with a passion for working with vulnerable populations. Your excellent communication and interpersonal skills will enable you to build strong relationships with members, colleagues, and community partners.
