The South West Yorkshire Partnership NHS Trust is seeking a highly skilled and dedicated Health Care Support Worker to join our Intensive Home Based Treatment Team. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals with mental health needs.
Main Responsibilities- Under the supervision of registered healthcare professionals, you will provide psychological and physical care to a defined group of individuals, ensuring their safety and well-being.
- You will work collaboratively with a multidisciplinary team to deliver planned care, maximizing the recovery and rehabilitation potential of service users.
- Develop and maintain meaningful therapeutic relationships with individuals, adopting a polite, positive, and empathetic approach to communication.
- You will demonstrate an understanding of care and treatment programs, enabling you to instruct, advise, and guide service users, family, and carers.
- Exercise initiative, decision-making, and prioritization skills to ensure effective delivery of care and services.
- A satisfactory sickness record over the previous 2 years.
- Able to fulfill Occupational Health requirements for the post, with reasonable adjustments if necessary.
- Must be willing and able to undertake all appropriate Trust mandatory training requirements and subsequent refresher training relevant to the area.
- Demonstrate excellent knowledge of service user safety, including prevention and management of violence and aggression, moving and handling, etc.
- In providing personal care and related procedures for those with health/social needs.
- In working in a caring environment at a band 2 or equivalent level of competence.
- In dealing with the public.
- In working as an effective team member.
- Ability to work effectively, professionally, and independently or as part of a multidisciplinary team.
- GCSE A-D or grade 3-9 in Maths and English or equivalent.
- Apprenticeship level 3 or equivalent, e.g., RQF (Regulated Qualification Framework) NVQ level 3 in Health and Social Care or equivalent.
- Care Certificate or required to achieve within 12 weeks of commencement.
- Level 4 units in Health & Social Care.
- Experience of the speciality area.
- Willingness to undertake further study towards level 4 qualification.
- Responsive attitude and approach.
- Dress appropriately for the environment.
- A current driving license and access to a car during the working day is essential (reasonable adjustments will be considered for any applicants who are unable to drive due to a disability).
- Demonstrates excellent customer care skills.
- Enthusiastic, able to work flexibly on a 24hr shift pattern to meet the needs of service users.
- Demonstrates safe decision-making skills commensurate with role.
- Excellent communicator.
