Job Summary

The South West Yorkshire Partnership NHS Trust is seeking a highly skilled and dedicated Health Care Support Worker to join our Intensive Home Based Treatment Team. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals with mental health needs.

Main Responsibilities
  • Under the supervision of registered healthcare professionals, you will provide psychological and physical care to a defined group of individuals, ensuring their safety and well-being.
  • You will work collaboratively with a multidisciplinary team to deliver planned care, maximizing the recovery and rehabilitation potential of service users.
  • Develop and maintain meaningful therapeutic relationships with individuals, adopting a polite, positive, and empathetic approach to communication.
  • You will demonstrate an understanding of care and treatment programs, enabling you to instruct, advise, and guide service users, family, and carers.
  • Exercise initiative, decision-making, and prioritization skills to ensure effective delivery of care and services.
Requirements
  • A satisfactory sickness record over the previous 2 years.
  • Able to fulfill Occupational Health requirements for the post, with reasonable adjustments if necessary.
  • Must be willing and able to undertake all appropriate Trust mandatory training requirements and subsequent refresher training relevant to the area.
  • Demonstrate excellent knowledge of service user safety, including prevention and management of violence and aggression, moving and handling, etc.
  • In providing personal care and related procedures for those with health/social needs.
  • In working in a caring environment at a band 2 or equivalent level of competence.
  • In dealing with the public.
  • In working as an effective team member.
  • Ability to work effectively, professionally, and independently or as part of a multidisciplinary team.
  • GCSE A-D or grade 3-9 in Maths and English or equivalent.
  • Apprenticeship level 3 or equivalent, e.g., RQF (Regulated Qualification Framework) NVQ level 3 in Health and Social Care or equivalent.
  • Care Certificate or required to achieve within 12 weeks of commencement.
Desirable Qualifications and Experience
  • Level 4 units in Health & Social Care.
  • Experience of the speciality area.
  • Willingness to undertake further study towards level 4 qualification.

  • A current driving license and access to a car during the working day is essential (reasonable adjustments will be considered for any applicants who are unable to drive due to a disability).
