Chemistree Solutions Limited is seeking a dedicated and skilled Diabetes Specialist Nurse to join our team. As a key member of our healthcare team, you will play a vital role in supporting healthcare professionals and patients in making informed decisions about treatment pathways in Diabetes.
About the Role
This is a unique opportunity to work in partnership with the NHS to deliver a nursing programme focused on driving best practice, delivering excellent care, and improving patient outcomes. You will be responsible for working closely with patients and practices to assist healthcare professionals in delivering the most up-to-date care for their patients.
Your Key Responsibilities
- Support healthcare professionals in delivering high-quality Diabetes care
- Work closely with patients and practices to assist in making informed decisions about treatment pathways
- Develop and deliver educational programmes to support healthcare professionals and patients
- Collaborate with the NHS to deliver a nursing programme focused on driving best practice and improving patient outcomes
Requirements
- Current NMC registration
- Diabetes Diploma or equivalent
- Full UK driving licence
- A minimum of 3 years' experience managing Diabetes within primary care
- Strong knowledge and skills in the provision of education, support, and advice to people with Type 2 Diabetes
- IT literacy, including primary care systems
