Job Title: Diabetes Specialist Nurse - Primary Care Expert

We are working with a National Service provider to recruit a Diabetes Specialist Nurse to join their team. The successful candidate will work within primary care, conducting clinics in community settings and providing education, support, and advice to people with Type 2 Diabetes.

Key Responsibilities:

* Identify sub-optimally controlled patients with Type 2 diabetes
* Optimise oral therapy and conduct point of care testing
* Refer for specialist insulin treatment where necessary
* Exercise corporate nursing responsibilities

Requirements:

* Diabetes diploma or equivalent
* Minimum of 3 years experience managing diabetes within primary care
* Knowledgeable and confident in the provision of education, support, and advice to people with Type 2 Diabetes
* Full driving licence
* Current NMC registration
* IT literacy, to include primary care systems

Benefits:

* £45,000 annual salary + package
* Annual bonus incentive scheme (worth up to 10% of annual salary)
* Car allowance or company car (depending on contract) + fuel card for business miles
* Company pension

About the Role:

This is a hybrid-working position, with a mix of clinic work and home-based activities. The successful candidate will be working with a National Service provider to develop a nationwide Diabetes team.
