About the Role

We are seeking a highly skilled and motivated Community Nurse to join our team in Surrey Downs Health and Care. As a Community Nurse, you will be responsible for providing high-quality patient care in a community setting, working closely with our multidisciplinary team to deliver excellent patient outcomes.

Key Responsibilities
  • Assess and manage patient care needs in a community setting
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
  • Provide education and support to patients and families to promote health and well-being
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
  • Participate in quality improvement initiatives to enhance patient care and services
Requirements

To be successful in this role, you will need:

  • Registration with the Nursing and Midwifery Council (NMC)
  • Valid driving license and access to a vehicle for work purposes
  • Teaching qualification and experience in team management and multi-disciplinary working
  • Relevant post-registration experience in a community care setting
