About the Role

We are seeking a highly motivated and enthusiastic registered nurse to join our innovative community nursing team at Leatherhead Primary Care Network. As a Community Health Practitioner, you will be responsible for providing high-quality care to patients in their own homes, working closely with our multidisciplinary team to deliver person-centered care.

Key Responsibilities

  • Assess and deliver care to patients in their own homes, working closely with our multidisciplinary team
  • Develop and implement holistic care plans, taking into account the individual needs and preferences of each patient
  • Work closely with our community hospitals and specialist teams to ensure seamless care transitions
  • Participate in the development and implementation of innovative practice, contributing to the growth and development of our team

Requirements

  • Registered Adult Nurse with NMC
  • Valid Driving Licence (to drive within UK)
