Job Overview

We are seeking a skilled and compassionate Band 5 Community Nurse to join our Primary Care Network team. As a Community Nurse, you will play a vital role in delivering high-quality patient care and contributing to the development of innovative practice.

Main Responsibilities

  • Assess care needs and develop, implement, and evaluate holistic care packages.
  • Work closely with the community hospitals and specialist teams to ensure a whole system approach to care.
  • Provide ongoing care and support to patients in their homes, care homes, and through in-reach into hospitals.
  • Deploy the most appropriate staff member(s) in a coordinated and timely way to best use community resources.
  • Reduce risk of admission for patients.

Person Specification

  • Registered Adult Nurse with NMC.
  • Valid Driving Licence (to drive within UK).
  • Relevant post-registration experience and community experience.
  • Ability to assess, deliver, and evaluate quality care.
  • Effective communicator.
