We are seeking a highly skilled and experienced Band 7 Physiotherapist or Occupational Therapist to join our integrated Medical HSEP (Health Services for Elderly People) therapy team at Royal Free London NHS Foundation Trust.
Key Responsibilities- Provide clinical leadership to the Medical HSEP team and participate in daily operational activities and management.
- Develop and implement strategies for improving the quality of care for older patients and those living with frailty and dementia.
- Collaborate with multidisciplinary teams to ensure safe discharges and provide excellent quality of care.
- Participate in quality improvement projects and assist in setting new directions for the service.
- Lead on and participate in the development of policies and procedures for the Medical HSEP service.
- Post-registration experience in in-patient Medical HSEP and Frailty with examples.
- Extensive experience working in an acute healthcare setting and complex discharge planning with examples.
- Effective written and verbal communication skills to a range of audiences with examples.
- Demonstrate advanced clinical reasoning skills and management of complex cases with examples.
- Able to work independently as an autonomous practitioner with examples.
- Ability to work as part of a team including MDT with examples.
- Good organisational skills with examples.
- Reflect and critically approve own performance with examples.
- Ability to lead and motivate self and team with examples.
- Effective time management skills and ability to prioritise case load and managerial tasks with examples.
- Awareness of current health issues.
- IT skills / computer literate.
- Able to maintain judgment and make decisions under pressure with examples.
- Excellent presentation and training skills with examples.
- Clear vision of role/commitment to specialty.
- Demonstrate ability to meet Trust Values in a logical manner with proficient language.
- Degree in Physiotherapy or Occupational Therapy.
- Current HCPC registration.
- Demonstration of commitment to lifelong learning.
- Demonstration of interest in Frailty and team specialty through attendance at relevant courses.
