Frailty and Medical HSEP Specialist

About the Role
We are seeking a highly motivated and experienced Frailty and Medical HSEP Specialist to join our dedicated team at Royal Free London NHS Foundation Trust. This is an exciting opportunity to make a real difference in the lives of patients experiencing frailty and complex medical needs.
Key Responsibilities
- Provide expert clinical care to patients within an acute healthcare setting, focusing on Medical HSEP (Hospital Episode Statistics) and Frailty management.
- Develop and implement comprehensive discharge plans for patients with complex needs, ensuring seamless transitions between care settings.
- Collaborate effectively with multidisciplinary teams (MDTs), including physicians, nurses, social workers, and therapists, to deliver holistic patient care.
- Contribute to the ongoing development and improvement of clinical pathways and protocols related to Frailty and Medical HSEP.
Essential Skills and Experience
- Proven post-registration experience in inpatient Medical HSEP and Frailty, with a strong track record of delivering high-quality patient care.
- Extensive experience working within an acute healthcare environment, demonstrating expertise in complex discharge planning.
- Exceptional communication skills, both written and verbal, enabling effective interaction with patients, families, and colleagues from diverse backgrounds.
- Advanced clinical reasoning abilities and a demonstrated capacity to manage complex cases effectively.
- A commitment to continuous professional development and staying abreast of current best practices in Frailty and Medical HSEP management.
Desirable Skills and Experience
- Experience in audit processes, contributing to quality improvement initiatives within the healthcare setting.
- Proven ability to coach, supervise, and mentor staff members, fostering a culture of learning and professional growth.
- Demonstrated leadership qualities and experience in first-line management roles.
- Experience in acute medicine, specifically within the areas of Medical HSEP and Frailty.
