Job Overview
At Cockfosters Medical Centre, we are seeking an experienced and skilled nurse to join our team as an Advanced Practice Nurse. This is a challenging and rewarding role that requires a high level of clinical expertise and leadership skills.

Key Responsibilities
**Clinical Practice**: Provide high-quality nursing care to patients, including identifying, managing, and supporting those at risk of developing long-term conditions. Implement evidence-based practice, adhering to NICE guidelines and the NSF.

**Vaccination and Travel Medicine**: Offer vaccination and travel medicine services to patients, ensuring they receive the necessary care and advice.

**Wound Care and Cervical Smears**: Provide wound care, including ulcer and Doppler assessments, as well as cervical smear tests, maintaining accurate records and ensuring timely follow-up.

**Chronic Disease Management**: Develop and implement chronic disease clinics, delivering patient care and referring patients to secondary or specialist care as needed. Maintain accurate clinical records and ensure effective use of read codes.

**Leadership and Teamwork**: Support the clinical team with safeguarding matters, prioritizing health issues, and intervening appropriately. Recognize, assess, and refer patients presenting with mental health needs, ensuring seamless care coordination.

You will be responsible for maintaining accurate records, managing emergency drug kits, and ensuring the effective delivery of health promotion initiatives.
