Job Description:

A leading healthcare organization in Stoke-on-Trent is seeking an advanced practice healthcare professional to join its team. The successful candidate will be responsible for delivering high-quality patient care, developing care plans, and collaborating with multidisciplinary team members.

  1. Provide comprehensive nursing services, including wound management, immunizations, and health screenings.
  2. Conduct thorough patient assessments, develop care plans, and implement evidence-based practices.
  3. Collaborate with General Practitioners and other healthcare professionals to ensure seamless continuity of care.
Key Responsibilities:
  1. Develop and implement health promotion and education programs.
  2. Deliver individualized care to patients with complex needs.
  3. Collaborate with multidisciplinary team members to improve patient outcomes.
Requirements and Qualifications:
  • Registered Nurse (RGN) with an active NMC registration.
  • Prior experience in primary care or community healthcare settings is highly desirable.
  • Competence in administering child and adult immunizations is essential.
