Job Description

Job Summary:

The Dorset Clinical Commissioning Group is seeking a highly skilled Community Health Specialist to join our team. As a Community Health Specialist, you will play a vital role in providing high-quality person-centered care to individuals with learning disabilities.

Key Responsibilities:

  • Develop and implement a plan to increase screening uptake, reflecting variations across Dorset, and inform the development of a sustainable delivery model.
  • Provide supervision, mentorship, training, and clinical support to students and trained practitioners.
  • Collaborate with the Dorset Screening programme service to support people with learning disabilities access screening and improve health inequalities.
  • Work with Clinical Teams for Learning Disabilities (CTLD) and Primary Care to support access to national screening programs for patients with learning disabilities.

Requirements:

  • Registered practitioner, public health professional, or lived experience peer worker to degree/diploma level supplemented by post-registration training.
  • Registration diploma level specialist training and/or short courses or demonstrable extensive experience in the relevant specialty.
  • Experience at Practitioner Band 5 level or equivalent if non-NHS background.
  • Demonstrable knowledge of assessment and health promotion interventions in the area of specialism.
  • Ability to prioritize and organize workload effectively.

Desirable Criteria:

  • Learning and Assessing in Practice Qualification or equivalent practice assessors training.
  • Experience of developing specialist programs of care for individuals or groups of patients/clients and providing highly specialist advice.
  • Able to demonstrate specialist clinical reasoning skills to assimilate information and make clinical judgments.

Personal Qualities:

  • Able to communicate effectively at different levels of the organization and with staff, patient/service users, visitors, or external organizations.
  • Able to overcome barriers to understanding where there are physical or cognitive barriers.
  • Experience of planning and organizing complex activities, such as organizing own time and that of junior staff and learners.
  • Ability to evaluate care leading to improvement in quality standards and service improvement.

Managerial Experience:

  • Experience of providing clinical supervision and mentoring to junior staff.
