Job Title: District Nurse/Community Nursing Sister

We are seeking a highly skilled and experienced District Nurse or Community Nursing Sister to join our team in Poole Central locality. As a key member of our community nursing team, you will be responsible for leading the team and providing specialist care to adults with physical health needs.

Your role will involve case managing the provision of care within an individual's home or residential home, including palliative care and long term conditions management. You will work autonomously using your own initiative to deliver holistic care, and will be responsible for providing specialist advice to others regarding the management and care of patients/service users.

You will also be responsible for devising specialist programmes of care for other professionals to deliver, and for planning, implementing and reviewing health improvement programmes in a range of settings. Additionally, you will provide supervision, mentorship, training and clinical support to students and trained practitioners.

To be successful in this role, you will need to possess excellent leadership abilities, interpersonal skills and time management skills. You will also need to have experience in caseload management and be able to prioritise work.

This is a part-time role, with a requirement to work one weekend a month. Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.

Key Responsibilities:
  • Provide specialist care and interventions, assessments, treatments and managing patients on a caseload
  • Provide specialist advice to others regarding the management and care of patients/service users
  • Devising specialist programmes of care for other professionals to deliver
  • Planning, implementing and reviewing health improvement programmes in a range of settings
  • Providing supervision, mentorship, training and clinical support to students and trained practitioners
Requirements:
  • Registered practitioner to degree/diploma level supplemented by post registration diploma level specialist training and/or short courses or demonstrable extensive experience in the relevant specialty
  • Membership of the relevant Professional Body
  • Experience at Practitioner Band 5 level
  • Experience of specialist working
  • Experience of developing specialist programmes of care for an individual or groups of patients/clients and of providing highly specialist advice
Working Arrangements:

This role is part-time, with a requirement to work one weekend a month.

DBS Check:

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
