Job Title: District Nurse/Community Nursing Sister

We are seeking a highly skilled and experienced District Nurse or Community Nursing Sister to join our team in Poole Central locality. As a key member of our community nursing team, you will be responsible for leading the team and providing specialist care to adults with physical health needs.

The successful candidate will possess excellent leadership abilities, interpersonal skills, and time management skills. You will have experience in caseload management and be able to prioritize work. The role is diverse, challenging, and extremely rewarding, with opportunities for professional growth and development.

Main Responsibilities:

  • Provide high-quality person-centered care to patients, considering their safety, privacy, and dignity.
  • Deliver specialist care and interventions, assessments, treatments, and manage patients on a caseload as part of the wider multi-disciplinary care team.
  • Provide specialist advice to others regarding the management and care of patients/service users.
  • Devising specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.
  • Plan, implement, and review health improvement programmes in a range of settings.
  • Provide supervision, mentorship, training, and clinical support to students and trained practitioners.

Requirements:

  • Registered practitioner to degree/diploma level supplemented by post-registration diploma level specialist training and/or short courses or demonstrable extensive experience in the relevant specialty.
  • Membership of the relevant Professional Body.

Desirable Qualifications and Experience:

  • Experience at Practitioner Band 5 level.
  • Experience of specialist working.
  • Experience of developing specialist programmes of care for an individual or groups of patients/clients and of providing highly specialist advice.

Working Arrangements:

This is a part-time role, with one weekend per month. Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
