Job Summary

We are seeking a highly skilled and dedicated Community Nurse to join our team at Cambridgeshire and Peterborough NHS Foundation Trust. As a Community Nurse, you will play a vital role in providing high-quality care to patients in their own homes, working closely with other healthcare professionals to ensure seamless care delivery.

Main Responsibilities
  • Assess, plan, and implement care for patients in their own homes, ensuring their needs are met and their safety is prioritized.
  • Work collaboratively with other healthcare professionals, including GPs, district nurses, and social workers, to ensure comprehensive care delivery.
  • Provide education and support to patients, carers, and other healthcare professionals to promote health and well-being.
  • Conduct home visits, assessing patients' needs and developing care plans to meet their individual requirements.
  • Work autonomously, making decisions and taking responsibility for patient care, while also being part of a multidisciplinary team.

Requirements
  • RGN level 1 and 2 qualifications.
  • Experience of working in the community, preferably in a similar role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals.
  • Ability to work flexibly, including evenings and weekends, to meet the needs of patients.
  • Full UK driving license and access to a vehicle, as regular travel is required.
