Job Overview

We are seeking a Band 4 Community Nursing Associate to join our dedicated team, focusing on delivering exceptional care to patients with clinical health requirements.

This position offers an opportunity to refine and advance your clinical skills in a community setting, encompassing areas such as wound care, discharge assistance, diabetes management, catheter care, and support for individuals with chronic conditions living at home.

The successful candidate will provide high-quality, evidence-based nursing guidance, support, and care for individuals with physical health challenges and their caregivers within a multidisciplinary environment. Responsibilities will include the assessment, planning, execution, and evaluation of nursing care in accordance with established NMC standards specific to this role, along with a comprehensive understanding of relevant local and national policies and regulations governing service delivery.

Additionally, the post holder will identify and contribute to the training needs of junior unregistered staff, ensuring adherence to principles of equality, diversity, and rights in line with best practices and legislation. Effective communication pathways will be established, and the individual will operate within their defined scope of responsibility.

Key Responsibilities

Collaborating as a vital member of the Community team, you will undertake delegated responsibilities to meet the needs of service users. Training and supervision will be provided by registered professionals, and you will be expected to adhere to a care plan tailored to the specific needs of each service user.

Job Responsibilities

  • Responsible for the assessment, planning, implementation, and evaluation of evidence-based care, including health promotion for designated service users/groups, ensuring compliance with Trust timelines and NMC standards.
  • Participate in establishing quality standards, including monitoring and reviewing practices in accordance with current clinical guidelines and policies.
  • Ensure understanding and adherence to professional roles and responsibilities under the Care Programme Approach.
  • Comply with the requirements of the Mental Health Act 2007.
  • Promote involvement of caregivers and service users in service provision.
  • Encourage recovery, discovery, and independence, considering risk assessment and management.
  • Establish therapeutic relationships with service users and implement evidence-based therapeutic interventions, maintaining appropriate boundaries as per the Nursing and Midwifery Council's professional code of conduct.
  • Demonstrate care and compassion in all interactions.
  • Contribute to and work towards the service and organizational aims and objectives.
  • Facilitate high standards of care through the development of written care plans for clients, validated by senior staff.
  • Maintain competency in specific nursing skills required for the role, as outlined by NMC standards for Nursing Associates and local policies.
  • Administer medications with precision, ensuring correct dosage, timing, route, and documentation in accordance with Trust policy.

Education / Qualifications

Essential

  • Registered Nursing Associate on the NMC register
  • Minimum Level 5 Foundation Degree specific to Nursing Associate Practice.
  • GCSEs Grade A - C in Maths and English or equivalent

Desirable

  • QCF in Care / Health & Social Care Level 2/3 or equivalent experience
  • Completed Care Certificate

Experience

  • Experience in inpatient services and collaboration within a multi-disciplinary team
