About the Role

We are seeking a skilled and compassionate Community Triage Clinician to join our team at Norfolk Community Health and Care NHS Trust. As a key member of our triage team, you will play a vital role in assessing and prioritizing patient referrals, ensuring timely and effective care for our community.

Key Responsibilities

  • Assess and prioritize patient referrals, using clinical judgment to determine the most appropriate course of action.
  • Develop and implement individualized care plans, working closely with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Provide high-quality patient education and support, empowering individuals to manage their health and well-being.
  • Liaise with multidisciplinary teams, including GPs, social services, and other healthcare professionals, to ensure comprehensive care and support.
  • Contribute to the development and implementation of clinical policies and procedures, ensuring best practice and quality care.

Requirements

  • Diploma or degree in nursing or a related field.
  • Current NMC registration.
  • ENB998 or equivalent mentorship preparation.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Strong clinical judgment and decision-making skills.
  • Ability to prioritize tasks and manage time effectively.
