About the Role

We are seeking a skilled and compassionate Community Triage Clinician to join our team at Norfolk Community Health and Care NHS Trust. As a key member of our triage team, you will play a vital role in assessing and prioritizing patient referrals, ensuring that patients receive the right care at the right time.

Key Responsibilities

  • Assess and prioritize patient referrals, using clinical judgment to determine the most appropriate course of action.
  • Develop and implement individualized care plans, working closely with patients, families, and healthcare teams to ensure seamless care coordination.
  • Communicate effectively with patients, families, and healthcare professionals, using empathy and tact to address complex needs and concerns.
  • Collaborate with multidisciplinary teams to ensure comprehensive care, including referrals to specialist services and community resources.
  • Contribute to the development and implementation of care pathways, ensuring that patients receive evidence-based care.
  • Participate in quality improvement initiatives, using data and feedback to inform practice and improve patient outcomes.

Requirements

  • Diploma or degree in nursing or a related field.
  • Current registration with the Nursing and Midwifery Council (NMC).
  • Experience in community nursing or a related field, with a strong understanding of triage principles and practices.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work autonomously, with a high degree of independence and self-motivation.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
