About the Role

We are seeking a skilled and compassionate Community Triage Clinician to join our team at Norfolk Community Health and Care NHS Trust. As a key member of our triage team, you will play a vital role in assessing and prioritizing patient referrals, ensuring that patients receive the right care at the right time.

Key Responsibilities

  • Assess and prioritize patient referrals, using clinical judgment to determine the most appropriate course of action.
  • Develop and implement individualized care plans, working closely with patients, families, and healthcare teams to ensure seamless care coordination.
  • Communicate effectively with patients, families, and healthcare professionals, using empathy and tact to address complex needs and concerns.
  • Collaborate with multidisciplinary teams to ensure comprehensive care, including liaison with GPs, social services, and other healthcare professionals.
  • Participate in clinical supervision and peer review to maintain high standards of practice and continuous professional development.

Requirements

  • Diploma or degree in nursing or a related field, with current NMC registration.
  • ENB998 or mentorship preparation, or equivalent.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Highly motivated and able to work autonomously, with a strong commitment to patient-centered care.
  • Ability to use own initiative, with good observational and reporting skills.
