Job Summary

We are seeking a skilled and compassionate Community Triage Clinician to join our team in the North Locality Hub. As a Community Triage Clinician, you will play a vital role in assessing patients' needs, determining the priority of care, and processing referrals accordingly. This will involve contacting patients and other professionals via telephone to provide an initial assessment and advice.

Main Duties
  • Process referrals by evaluating the information received and allocating to the appropriate service, indicating the priority response necessary.
  • Document triage notes to ensure patients' records are complete and for audit purposes.
  • Use information technology to process referrals for the 80% triaging role.
  • Liaise and communicate with the general public, other healthcare professionals, and multi-agencies on a daily basis.
  • Assess, prescribe, and deliver care packages for patients with long-term conditions, palliative care, and rehabilitation needs to achieve quality of life and independence where possible.
  • Work within the integrated team to facilitate early discharge from hospital and prevent unnecessary admission to hospital.
  • Work with all healthcare professionals and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
