About the Role

We are seeking a skilled and compassionate Community Triage Clinician to join our team in the North Locality Hub. As a key member of our triage team, you will be responsible for assessing patient needs, determining priority of care, and processing referrals accordingly.

Key Responsibilities

  • Process referrals by evaluating information received and allocating to appropriate service, indicating priority response necessary.
  • Document triage notes to ensure patient records are complete and for audit purposes.
  • Liaise and communicate with general public, healthcare professionals, and multi-agencies on a daily basis.
  • Assess, prescribe, and deliver care packages for patients with long-term conditions, palliative care, and rehabilitation needs.
  • Work within integrated team to facilitate early discharge from hospital and prevent unnecessary admission.

Requirements

  • Diploma or degree in nursing or equivalent.
  • Current NMC registration.
  • ENB998 or mentorship preparation.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
