Lead Care and Support Practitioner

MioCare Group is seeking a Lead Care and Support Practitioner to join our team. In this role, you will be responsible for leading a team of care workers in providing high-quality care and support to individuals in need of extra care. You will work closely with healthcare professionals, including GPs, nurses, and physiotherapists, to deliver holistic care and support. Our ideal candidate has a strong leadership style, excellent communication skills, and a passion for delivering exceptional care.
Key Responsibilities
The successful candidate will be responsible for ensuring that all duties directed by the Registered Manager and detailed in the service user's plan and risk assessments are carried out in full. You will update and review care plans regularly, participating in the completion of risk assessments and care plans to ensure the needs of service users are met. We expect you to work collaboratively with other professionals to achieve the best outcomes for our service users.
What We Offer
We offer a competitive salary of £30,000 - £40,000 per annum, depending on experience.
