We are seeking a skilled and compassionate Mental Health Practitioner to join our team at Devon Partnership NHS Trust. As a Community Practitioner, you will play a vital role in delivering high-quality mental health services to children and young people in our community.
About the RoleThis is a Band 5 developmental post, offering a unique opportunity for a clinician to develop their knowledge and skills in a dynamic and expanding multi-disciplinary team. You will work closely with our experienced professionals to provide assessment, risk planning, and treatment plans to individuals with mental health difficulties and their carers.
The successful candidate will have a qualification in mental health or occupational therapy and will be working in a 24/7 service, including night shifts on a rota basis. We are looking for someone with significant experience in a mental health setting, preferably at Band 5 level, who can demonstrate sound theoretical knowledge of mental health practice.
Main Responsibilities- Deliver high-quality mental health services, focusing on triage, assessment, and recovery-focused planning, referral, and signposting treatment to individuals within the general hospital.
- Impart complex and sensitive information to patients and families/carers regarding assessment, care planning, treatment, and review.
- Facilitate fair access to social care services and promote social inclusion regardless of age, ethnicity, gender, sexual orientation, or disability.
- Undertake risk assessments and determine the appropriate course of action, ensuring consultation with senior colleagues as necessary.
- Undertake bio-psycho-social assessments and develop a working formulation and diagnosis, leading to the development of personalized care packages.
- An appropriate up-to-date professional mental health qualification, such as RMN, OT, or Social Worker.
- Training to level 2 in Safeguarding children and young people.
- Able to work 24-hour shift patterns, including nights, in line with service requirements.
