Care Manager for Complex Needs

We are seeking a skilled Care Manager to lead our team in providing high-quality care services to individuals with complex needs. As a key member of our care team, you will be responsible for overseeing daily care operations, scheduling shifts, and ensuring person-centered care is delivered.
The ideal candidate will have a strong background in health and social care, with experience managing care services in a community setting. They will also possess excellent leadership skills, with the ability to foster positive relationships and guide teams in residential care settings.
As a Care Manager, you will be responsible for conducting regular appraisals, managing staff performance, and coordinating multi-disciplinary meetings to support residents' needs. You will also be involved in managing referrals, overseeing medication administration, and ensuring compliance with regulatory standards.
In addition to your clinical expertise, you will be expected to contribute to a positive and inclusive team culture, promoting diversity and respect for all individuals. - NVQ Level 2/3 in Health and Social Care
- Proven experience managing care services within a community setting for individuals with complex needs

- Proven track record in managing complex care packages and supporting individuals with mental health conditions, personality disorders, autism, and learning disabilities
