Job Summary

We are seeking a skilled Care Coordinator to join our team at North Shoebury Surgery. As a Care Coordinator, you will play a vital role in ensuring that patients receive the best possible care and service. You will work closely with our clinical team to coordinate patient care, provide support and guidance, and help patients navigate the healthcare system.

Main Responsibilities

  • Coordinate patient care and support services, including access to healthcare resources and information.
  • Work closely with the clinical team to ensure timely and efficient patient care.
  • Provide guidance and support to patients and their families, helping them to understand their healthcare options and make informed decisions.
  • Develop and maintain relationships with local healthcare providers and community resources, ensuring that patients have access to the care and support they need.
  • Contribute to the development and implementation of patient care plans, working closely with the clinical team to ensure that patients receive the best possible care.

Requirements

  • English Grade C and above
  • Maths Grade C and above
  • Diploma Level 2 in Health and Social Care or equivalent qualification
  • Experience of working in healthcare, the voluntary or community sector, to support vulnerable groups
  • Strong listening and communication skills
  • Familiar with local resources and services, including how to access them
  • Motivated by helping people, with care and empathy
  • Able to deliver person-centred support in a non-judgmental way
  • Experience of completing holistic person-centred care planning assessments
  • Strong IT skills
  • Cross-site working
