Job Summary
Primary Care Careers is seeking a skilled Care Co-ordinator to join our team at North Shoebury Surgery. This is a fantastic opportunity to work closely with our clinical team and ensure that patients receive the best possible care and service.
Main Responsibilities
- Support the clinical directors in coordinating key activities, including access to services, advice, and information, to ensure timely, efficient, and patient-centred health and care planning.
- Contribute to digital initiatives and coordinate patient journeys through primary care.

Located in Shoeburyness, our surgery provides a range of NHS primary care services. We aim to provide an effective, efficient, and fair healthcare service to all our patients. Our professional team works well together, and we offer ongoing training and the NHS pension scheme with a 20% employer contribution.
Person Specification
Qualifications
- English Grade C and above
- Maths Grade C and above
- Ideal qualifications include a Diploma Level 2 in Health and Social Care or an equivalent qualification.
Experience
- Experience of working in healthcare, the voluntary or community sector, to support vulnerable groups.
- Strong listening and communication skills.
- Familiarity with local resources and services, including how to access them.
- Motivated by helping people with care and empathy.
- Able to deliver person-centred support in a non-judgmental way.
- Experience of completing holistic person-centred care planning assessments.
- Strong IT skills.
- Cross-site working.
