Job Summary

Main Responsibilities

  • Support the clinical directors in coordinating access to services, advice, and information.
  • Ensure health and care planning is timely, efficient, and patient-centred.
  • Support digital initiatives and coordinate patient journeys through primary care.

Person Specification

Qualifications

  • English Grade C and above.
  • Maths Grade C and above.
  • Ideal qualifications include a Diploma Level 2 in Health and Social Care or an equivalent qualification.

Experience

  • Experience of working in healthcare, the voluntary or community sector, to support vulnerable groups.
  • Strong listening and communication skills.
  • Familiar with local resources and services, including how to access them.
  • Motivated by helping people, with care and empathy.
  • Able to deliver person-centred support in a non-judgmental way.
  • Experience of completing holistic person-centred care planning assessments.
  • Strong IT skills.
  • Cross-site working.
