Qualifications

Essential

  • English Grade C and above
  • Maths Grade C and above
  • Diploma Level 2 in Health and Social Care or equivalent qualification

Experience

Essential

  • Experience in healthcare, the voluntary or community sector, supporting vulnerable groups
  • Strong listening and communication skills
  • Familiarity with local resources and services, including access to them
  • Motivated by helping people with care and empathy
  • Able to deliver person-centred support in a non-judgmental way
  • Experience in completing holistic person-centred care planning assessments
  • Strong IT skills
  • Cross-site working
