Key Responsibilities- Assist the AED Step Care team in delivering holistic care to patients, promoting their physical and emotional wellbeing.
- Contribute to and interpret care plans accurately and consistently, ensuring seamless communication with the multi-disciplinary team.
- Carry out delegated care tasks, including supported meals and skills sessions, to support patients' recovery.
- Assess and promote good physical health, raising concerns with the team as necessary.
- Observe and report any changes in patients' conditions to the key Nurse and the team.
- Communicate effectively with relatives and carers, providing support and guidance as needed.
- Liaise with other relevant individuals involved with the patient, as appropriate.
- Write accurate records of care activities and communications with patients, relatives, and carers in the patient's notes.
- Support patients with individual needs and choices, promoting autonomy and independence.
- Provide formal verbal and/or written hand-over of patient care, ensuring continuity of care.
- Supply all clinical information within the required timescales on the electronic system RiO.
- Assist the team to identify and manage areas of risk in the home environment, safeguarding health and safety for patients and their families.
- Identify when patients are deteriorating and escalate concerns to avert emergencies.
- Undertake administrative duties as delegated by Senior Management Team.
