Mental Health Support Professional

Our team is looking for a highly skilled Mental Health Support Professional to join us in delivering exceptional care to working-age adults with complex needs.
In this role, you will be responsible for providing care-coordination services, conducting risk assessments, and developing care plans tailored to each individual's needs.
You will work closely with a multidisciplinary team to ensure that our service users receive comprehensive support and guidance.
Main Duties- To coordinate care services for a specified caseload, ensuring seamless delivery of interventions
- To assess and manage risks, identifying opportunities for prevention and early intervention
- To develop and implement care plans that address the unique needs and goals of each service user
The estimated salary for this position is £38,000 - £45,000 per annum, depending on experience and qualifications.
