Role Description

Sanctuary Personnel Ltd is seeking an experienced Social Worker to join their Malvern Team. As a member of the team, you will be responsible for working with adults with complex needs, providing intensive support, and collaborating with professionals to ensure a multi-agency approach.

  • Key Accountabilities:
  • Develop and implement person-centred plans to promote independence and well-being.
  • Collaborate with professionals to provide holistic support and address complex issues.
  • Conduct assessments and reviews to identify needs and develop support plans.
  • Provide emotional support and guidance to individuals and families.
  • Maintain accurate records and report on progress as required.

The successful candidate will have a Social Work qualification or equivalent, registration with the relevant health professional council/governing body (Social Care England), and excellent communication and interpersonal skills.
