Job Summary

The Norfolk Community Health and Care NHS Trust is seeking an experienced and highly motivated Registered Nurse to join our nursing leadership team in West Norfolk. As a Clinical Lead, you will play a key role in leading the development of nursing teams and ensuring the delivery of high-quality care to patients.

Main Duties and Responsibilities
  • Assess, prescribe, and deliver care packages for patients with long-term conditions, palliative care, and rehabilitation needs.
  • Work within the integrated team to facilitate early discharge from hospital and prevent unnecessary admission.
  • Lead the development of nursing teams and manage the differences in professional roles within integrated teams.
  • Work with all healthcare professionals and statutory/non-statutory agencies to provide a seamless, integrated service to patients.

  • Work with professional leads and maintain a clinical role (minimum 60%).
  • Ensure effective delivery of all safeguarding policies and procedures of NCH&C.

Key Requirements
  • RGN and current NMC registration.
  • 1st Level degree.
