About the Position:

We are seeking a dedicated and experienced Leaving Care Program Coordinator to join our team in Bury, Greater Manchester. As a key member of our Leaving Care Team, you will work closely with care leavers, their families, and other professionals to provide comprehensive support and guidance.

Main Responsibilities:

  • Lead and conduct comprehensive assessments and interventions related to Pathway Plans for care leavers.
  • Develop and maintain a thorough understanding of available services and resources for care leavers, ensuring they receive the best possible support.
  • Build strong relationships with care leavers, listening to their concerns and providing emotional support and practical advice.
  • Collaborate with other professionals, such as social workers, teachers, and health practitioners, to ensure a seamless service delivery.

Requirements:

  • NVQ Level 3 in Childcare, Family Support, or a related field (or equivalent qualification).
  • Willingness to pursue further education to achieve Level 4 or higher in Childcare, Family Support, or a comparable qualification.
  • Extensive experience working with care leavers, children, young people, families, and/or carers in a professional capacity.
  • Proven ability to build and maintain effective relationships with care leavers, their families, and other professionals.

Salary and Benefits:

The salary for this role is £36,648 - £38,223 per annum, depending on experience. You will also receive a range of benefits, including:
