Patient Care Coordinator

As a Patient Care Coordinator, you will be responsible for coordinating patient care activities, developing and maintaining patient records, and communicating with patients and families. You will also be involved in quality improvement initiatives and contribute to the development of policies and procedures.
The ideal candidate will have a strong understanding of healthcare systems and processes, as well as excellent communication and interpersonal skills. They will be able to work effectively with patients, families, and healthcare teams to deliver high-quality care.
We estimate that the salary for this position will be around £25,000 - £35,000 per annum, depending on experience.
Key Responsibilities:
- Coordinate patient care activities, including scheduling appointments and arranging transportation
- Develop and maintain accurate and up-to-date patient records
- Communicate with patients and families to provide education and support
- Contribute to quality improvement initiatives and policy development
Requirements:
- Diploma or certificate in a related field (e.g. healthcare, social work)
- 1+ year of experience in a patient care or healthcare role
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
