Job Summary

We are seeking a skilled and compassionate Care Coordinator in Complex Care to join our team at Kingdom Healthcare. This is a rewarding opportunity for a dedicated individual to make a positive impact on the lives of our clients with complex medical needs.

About the Role:

  • Assist patients with daily activities, mobility, and transfers.
  • Monitor vital signs, administer medication under supervision, and perform simple medical tasks.
  • Provide care for patients with complex needs, manage chronic conditions, and respond to medical emergencies.
  • Offer companionship, address social and emotional needs, and communicate effectively with patients.
  • Maintain accurate patient records and report concerns to supervising nurses or healthcare professionals.
  • Adhere to healthcare regulations, policies, and procedures to ensure patient safety and high-quality care.

Requirements:

  • Physical Stamina: necessary for assisting patients with mobility and performing physically demanding tasks.
  • Basic Medical Knowledge: understanding of medical terminology, basic healthcare procedures, and emergency response.

Previous healthcare experience is desirable but not essential. As a Care Coordinator in Complex Care, you will play a vital role in improving the quality of life for our clients with complex medical needs.
