Care Team Coordinator

We are seeking a highly skilled and experienced Home Support Manager to join our team at the Presbyterian Church in Ireland. As a key member of our care team, you will play a vital role in ensuring the delivery of high-quality care services to our residents.
About the RoleThe successful candidate will be responsible for providing leadership and management to our care staff, ensuring that they have the necessary skills and knowledge to provide exceptional care. You will also be responsible for coordinating care plans, administering medications, and supervising activities.
In addition, you will be required to lead or assist with daily prayers, implement Quality Assurance programs, and ensure compliance with professional standards.
Key Responsibilities- Care Planning and Coordination:
- Develop and implement individualized care plans for residents.
- Coordinate care activities and ensure that residents receive timely and effective care.
- Leadership and Management:
- Provide guidance and support to care staff, ensuring that they have the necessary skills and knowledge to provide exceptional care.
- Lead or assist with daily prayers and ensure adherence to rules and regulations.
- Quality Assurance and Compliance:
- Implement Quality Assurance programs and ensure compliance with professional standards.
- Monitor and evaluate care services to ensure that they meet the highest standards.
To be successful in this role, you will need:
- Qualifications and Experience:
- NVQ/QCF Level 2 Award in Health and Social Care and willingness to work towards Level 3.
- Minimum 2 years experience caring for older people, people with disabilities, or mental health needs.
- Personal Attributes:
- Good organisational, interpersonal, and leadership skills.
- Ability to work on own initiative and good reporting/recording skills.
- Caring and empathic disposition.
We offer a competitive salary of £25,000 - £30,000 per annum, depending on experience, as well as opportunities for professional development and growth.
