Job Description

The Diabetes Prevention Practitioner will work closely with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments. The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Main Responsibilities:

  • Deliver evidence-based interventions that focus on diet, nutrition, physical activity, and behaviour change
  • Facilitate group intervention sessions to support clients in achieving their goals and cultivating a safe and positive environment
  • Promote the Healthier You NHS Diabetes Prevention Programme internally and externally, and improve referral pathways between other services and partner organisations
