Preventive Care Practitioner

Job Overview:
We are seeking a passionate practitioner to join our bank team as part of our efforts to empower over a million people to change their behaviours and thrive. As a Disease Prevention Coach, you will play a vital role in improving lives through evidence-based interventions and facilitating group sessions.
Main Responsibilities:
- Evidence-Based Interventions: Deliver diet, nutrition, physical activity, and behaviour change interventions that focus on halting or reversing Type 2 Diabetes progression.
- Group Session Facilitation: Create a safe and positive environment for clients to achieve their goals and cultivate a sense of community.
- Programme Promotion: Promote the Healthier You NHS Diabetes Prevention Programme within Thrive Tribe and externally, enhancing collaboration with partner organisations and services.
- Ongoing Training: Engage in Behaviour Change Level 1 and 2 training and other relevant professional development opportunities.
- Quality Assurance: Work towards National Diabetes Prevention Programmes (NDPP) quality assurance framework criteria and targets.
