Job Overview

This exciting opportunity is for a skilled Health Visitor to join our team at HCRG Care Group in central Lancashire. As a Bank Health Visitor, you will be responsible for delivering high-quality care to families and children, focusing on the Healthy Child Programme and utilizing evidence-based practice.

Main Tasks and Responsibilities

  • Develop and implement individualized care plans for each child, based on their specific needs and requirements.
  • Conduct assessments of health needs and growth development in the 0-5 age range, using tools like the Family Health Needs Assessment and Early Help Assessment.
  • Collaborate with colleagues to ensure seamless delivery of care services, promoting a multi-disciplinary approach.

Requirements and Qualifications

  • A minimum SCPHN Health Visitor qualification, along with a valid UK driving license and access to a car for work purposes.
  • A proven ability to deliver high-quality care, with a focus on family-centered approaches and collaboration with colleagues.
