Clinical Lead Community Nurse

About the RoleWe are seeking a highly skilled Clinical Lead Community Nurse to join our team at Livewell Southwest. As a Clinical Lead, you will be responsible for providing planned and unplanned care to patients and their carers in a defined geographical area, working as part of a twenty-four hour District Nursing Service.
You will be required to assess, diagnose, plan, organize and review complex packages of care, including case conferences. Additionally, you will be responsible for undertaking comprehensive risk assessments of all situations associated with the care of patients to ensure nurses and carers' safety.
The ideal candidate will have a full driving licence and the ability to travel across Plymouth. You will also need to be able to respond to urgent calls in different parts of the city.

In this role, you will work alongside the Community Matron for Long Term conditions to monitor complex health needs of patients and reduce hospital admissions. You will also be responsible for implementing, participating and facilitating Clinical Supervision as per organizational Policy.
