Job Description

We are seeking a skilled Community Health Worker to join our team at Care Support.

The successful candidate will be responsible for addressing the causes of poor health and wellbeing in the community, working with individuals, groups, and communities to identify their strengths and needs.

Main Responsibilities:

  • Addressing the root causes of poor health and wellbeing in the community
  • Working with individuals, groups, and communities to build on their strengths and improve health and wellbeing
  • Identifying gaps in services and resources available to support health and wellbeing needs
  • Building relationships with local organisations and groups
  • Enabling individuals, networks, and communities to address unmet needs and improve health and wellbeing
  • Managing data and information and maintaining accurate records
  • Providing interventions that support health and wellbeing for individuals in specific settings or communities

Required Skills and Qualifications:

  • Willingness to work towards NVQ3 or QCF Level 3 in Community Health and Wellbeing
  • Basic Maths and English Skills
  • Good IT skills
  • Understanding of service provision in the care profession

Estimated Salary: £25,000 - £30,000 per annum, depending on experience.
