Personalised Support Worker for Individuals with Complex Needs

We are seeking a dedicated and compassionate Personalised Support Worker to join our team at Gray Healthcare Ltd. As a key member of our multi-disciplinary team, you will play a vital role in supporting individuals with complex needs, disabilities, and long-term conditions to live independently in their own homes.
With a keen interest in promoting independence and empowerment, you will work closely with our service users to develop and implement tailored support plans that meet their unique needs and aspirations. Your expertise in mental health and social care will be invaluable in creating a safe and supportive environment that fosters growth and development.
About the Role- Provide high-quality, person-centred support to individuals with complex needs, disabilities, and long-term conditions.
- Develop and implement tailored support plans that promote independence, empowerment, and well-being.
- Work collaboratively with our multi-disciplinary team to ensure comprehensive care and support.
- Contribute to ongoing assessments and reviews to ensure the effectiveness of support plans.
- Minimum 6 months' experience in mental health and social care, ideally within supported living or community settings.
- Proven ability to work with individuals with complex needs, including those with learning disabilities, autism, and sensory processing disorders.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and multidisciplinary teams.
- Current DBS check and willingness to undertake enhanced checks as required.
- A competitive salary of £12.03 per hour (36-hour contract) or £14.04 per hour (24-hour contract).
