Health and Wellbeing Practitioner Specialist

The Forward Trust is seeking a skilled Health and Wellbeing Practitioner to join our team at HMP Millsike. As a key member of our Connections Team, you will play a vital role in delivering our Health and Wellbeing Model of peer support across the prison.
About the RoleThis is an exciting opportunity to work in a new prison environment, focusing on promoting access to and delivery of workshops on general healthcare and wellbeing alongside substance misuse provider services. You will work closely with trained prisoner Health and Wellbeing Champions (HAWCs) to deliver a prison-wide health promotion calendar of events/activities for the year.
Key Responsibilities- Deliver comprehensive assessments and design implementation of SMART recovery plans.
- Carry out health & wellbeing interventions using motivational interviewing techniques in both 1:1 and therapeutic group settings.
- Organise workload, ensuring effective time management and prioritisation to meet targets.
To be successful in this role, you will require:
- Excellent experience of substance misuse and/or mental health work in a related field.
- Demonstrable experience of carrying out risk assessments.
- A thorough knowledge of and commitment to the 12-step process of recovery from addiction.
