Job Overview

We are seeking a skilled and compassionate Care Assistant Specialist to join our Reablement service in York. As a key member of our team, you will play a vital role in supporting individuals to manage their personal care within their own homes.

About the Role

The successful candidate will have excellent communication skills and be able to work independently as part of a close-knit team. You will be responsible for providing support and assistance to individuals who have been discharged from hospital or may have had an illness, crisis, fall at home or change in circumstances in relation to themselves or their careers.

  • Support individuals to maintain their independence and confidence
  • Work collaboratively with colleagues to achieve shared goals and outcomes
  • Provide emotional support and reassurance to individuals and their families
