Job Summary


We are seeking a Relief Me Time Facilitator to join our team. As a valued colleague, you will be flexible and committed to supporting people with learning disabilities to achieve their goals.



About the Role


The Relief Facilitator will work closely with the Me Time Team to provide person-centered plans, involving individuals in all aspects of planning and review processes. You will also be responsible for building positive relationships with people supported, their families, and the wider community.



Key Responsibilities



  • Contribute actively to individual and team performance by providing ideas and solutions.
  • Evaluate and ensure that plans are person-centered, with individuals fully involved in the planning and review process.
  • Be creative, dynamic, and adaptable when dealing with challenging scenarios related to individual needs.
  • Complete paperwork requirements on time and maintain high-quality reporting and recording.
  • Communicate with dignity and respect at all times.
  • Build rapport with individuals supported and the wider community, including family members.
  • Enable individuals to make decisions and choices, allowing them to access new opportunities aligned with their desired lifestyle.

Salary


We offer a competitive salary of £23,000 - £25,000 per annum, depending on experience.
