Community Healthcare Coordinator

We are seeking an experienced Paediatric Nurse Manager to join our team. As a key member of our community healthcare team, you will provide face-to-face support to children and their families, creating personalized care plans and working closely with a core team of carers to train and develop their clinical skills.
About You:
To succeed as a Paediatric Nurse Manager, you will need:
- An RSCN qualification with a minimum of 6 months' experience in the UK and an active NMC PIN
- A supportive management style that brings out the best in others
- Up-to-date experience of tracheostomy care and ventilators within the last 2 years
- Advanced communication skills to liaise effectively with patients, families, and the MDT
Key Responsibilities:
As a Paediatric Nurse Manager, your key responsibilities will include:
- Designing and delivering tailored care plans specific to individual clients' needs
- Effectively managing the MDT through strong communication and teamwork
- Providing ongoing monitoring, support, and assessment of clients' physical and physiological needs within your caseload
- Delivering nursing interventions, including the management of medicine delivery
- Building and maintaining sophisticated relationships with clients and their families
- Working closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care
