Job Summary:
Sanctuary Personnel Ltd is seeking a highly skilled Practice Improvement and Performance Manager to join our Adult Social Care Team. As a key member of our team, you will play a crucial role in ensuring the delivery of high-quality social care services to vulnerable adults.
Key Responsibilities:
- Safeguarding Adults: Support the service to ensure practice across teams ensures adults at risk of abuse are safeguarded.
- Needs Assessment and Meeting: Responsible for ensuring the assessment and meeting of eligible needs through a personalised approach of people, ensuring effective use of reablement resources to increase residents' independence and reduce demand on social care services.
- Proactive Approach: Take a proactive approach to ensure that vulnerable adults likely to be eligible for social care services do not fall between services and to ensure, in particular, close work with housing and corporate colleagues and key partners with vulnerable people.
- Service Planning and Delivery: Ensure services are planned and delivered in the light of developing the local and national priorities for personalisation, self-directed support for Social Care Services are integrated approaches across Adult Social Work.
- Relationship Building: Develop, promote and sustain positive and productive internal and external working relationships and partnerships with community, voluntary sector, government, professional bodies, and other public and private bodies.
- Expert Advice: In conjunction with the Head of Safeguarding, advise and support the Senior Management Team with expert knowledge of current and future adult social care practice issues and legislative context.
- Management Reviews: Undertake and supervise high-level management reviews, produce recommendations, and create action plans as needed to improve the quality of social work practice within ASW and attend the Serious Case Review/Safeguarding Adult Review Sub-Group.
Requirements:
- A social Work Qualification or equivalent.
- This post requires registration with the relevant health professional council/governing body (Social Care England).
