Practice Improvement and Performance Manager

Sanctuary Personnel Ltd, a leading recruitment agency, is seeking a highly skilled Practice Improvement and Performance Manager to join their Adult Social Care Team in Hillingdon. This permanent position offers a competitive salary of up to £62,595 per annum.

Key Responsibilities:
  • Support the service to ensure effective safeguarding of adults at risk of abuse across teams.
  • Lead the assessment and meeting of eligible needs through a personalized approach, ensuring effective use of reablement resources to increase residents' independence and reduce demand on social care services.
  • Take a proactive approach to ensure vulnerable adults likely to be eligible for social care services do not fall between services, working closely with housing and corporate colleagues and key partners.
  • Develop and implement services in line with local and national priorities for personalization and self-directed support for Social Care Services, integrating approaches across Adult Social Work.
  • Establish and maintain positive and productive internal and external working relationships and partnerships with community, voluntary sector, government, professional bodies, and other public and private bodies.
  • Provide expert knowledge and advice to the Senior Management Team on current and future adult social care practice issues and legislative context.
  • Conduct and supervise high-level management reviews, producing recommendations and creating action plans as needed to improve the quality of social work practice within ASW.
Requirements:
  • A Social Work Qualification or equivalent.
  • Registration with the relevant health professional council/governing body (Social Care England).
